Arizona Massage Therapy Licensing Requirements

Arizona requires a massage therapist license. The current standard Board pathway uses 700 classroom hours, an accepted national examination, and a Level One fingerprint clearance card.

Minimum education
700 hours
Approved education
The standard current Board path requires 700 classroom hours from a Board-recognized school. The statute supplies a 500-hour floor, while the current qualification rule governs the standard pathway at 700 hours.
Examination
The standard application framework uses an accepted national examination; the Board specifically identifies FSMTB or NCBTMB examination evidence for relevant applicants.
MBLEx relationship
MBLEx through FSMTB is an accepted examination path; NCBTMB examination evidence is also identified in current Board guidance for relevant applicants.
Application
Choose the Board's applicable regular, reciprocity, NCBTMB, or universal-recognition pathway; submit education, examination, identity, fingerprint-clearance, disclosure, and fee materials.

Background check or fingerprinting
A valid Level One fingerprint clearance card is required under current law and Board application guidance.

Arizona identifies separate reciprocity, NCBTMB, and universal-license-recognition paths. Conditions include credential duration, jurisdiction comparability, certification or education, residency for universal recognition, and other statutory requirements.

Renewal

Renewal and continuing education

Cycle
Licenses renew every two years.
Deadline model
The license expires every other year on the licensee's birthday.
Fees
Current rule lists $95 renewal and a $40 delinquent-renewal charge.
Continuing education
Complete 24 hours during the biennium, including at least 12 hours in person and no more than 12 by distance learning; Board-meeting attendance may account for no more than 3 hours. Qualifying education comes through the provider and institution routes in R4-15-302 rather than a Board course list.
Required subjects
No universal ethics-subject minimum was identified. Credit is capped at 4 hours of CPR and 4 hours of first aid, with no more than 8 combined, when instruction and instructor credentials satisfy R4-15-302; delivery and activity limits also apply.
Documentation
Renewal requires a CE-completion affirmation, and the December 2025 Board email directs applicants to upload CE certificates and a Level One fingerprint clearance card. R4-15-303 says the Board may annually and randomly select a minimum of 10 percent of active licenses, while the current Board CE page says up to 10 percent and the email describes auditing 10 percent of approved renewal applications each week. These official descriptions are not silently reconciled: public wording is limited to random audit, and the sampling percentage, population, and cadence require human review. No numeric licensee retention period was identified.
Carryover
Hours may not be carried over or reused.
Late renewal
Current rule lists a $40 delinquent-renewal charge. An expired credential may require additional conditions depending on elapsed time.
Inactive license
No general inactive path was established. After more than three years expired, reapplication is required and the Board may impose competency, internship, remediation, or examination conditions.

Scope of practice
Arizona defines massage therapy through therapeutic manipulation of soft tissue by specified manual methods and related modalities.
Protected titles
A person generally may not practice or hold out as a massage therapist without the state license, subject to statutory exemptions.
Restrictions
The credential does not independently authorize diagnosis, prescription, spinal manipulation, or acts reserved to another profession.
Municipal or local requirements
State law limits some local practitioner regulation but permits local regulation of massage establishments and general business matters within statutory bounds.
