Hot stone massage is a thermal adjunct in which warmed stones are used with or alongside an underlying massage modality. The stones do not define one universal massage system, cultural origin, or clinical mechanism.

04 · PRACTICE TODAY — PRESENT TENSE, NO HISTORY, NO OUTCOME PROMISES
How it is practiced and taught
Commercial practice commonly uses warmed stones as stationary heat sources or handheld tools. Stone material, heater design, temperature control, cleaning, barriers, and contact time vary and require technique-specific training.
A session includes heat-risk screening, temperature testing, communication, and sanitation. The client must be able to report discomfort and withdraw consent; heat does not establish detoxification or disease-level circulation claims.
SESSION ARC
- 01Heat-risk screen — Sensation, skin, vascular and neurologic context, medication, communication, and other heat risks are reviewed.
- 02Equipment and temperature check — Stones, heater, water, barriers, and test methods are prepared safely.
- 03Stationary or moving use — Warmed stones are placed or moved with conservative contact and continuous feedback.
- 04Removal and sanitation — Heat is stopped promptly for discomfort; equipment is cleaned between clients.

Multiple precedents
Heated objects appear in many practices, but the available record does not establish one universal ancient origin for generic hot stone massage.[S27]

06 · CAUTION LEDGER — THREE FIXED TIERS, EACH SOURCED, EACH DATED
What the profession documents
⚠ TIER 1 · ABSOLUTE CONTRAINDICATIONS
—Hot stones are not used when the client cannot reliably feel, communicate, or respond to heat, or over tissue where thermal injury risk cannot be controlled.[S27]
TIER 2 · SITE & PRESSURE CAUTIONS
—Hot contact is avoided over impaired sensation, fragile or inflamed skin, acute injury, vascular compromise, wounds, or other heat-sensitive tissue.[S27]
TIER 3 · POPULATION CAUTIONS
—Diabetes, neuropathy, vascular disease, pregnancy, medication effects, and communication limits warrant individualized caution, modification, or referral.[S27]

Required thermal-tool specialty education
Competence includes heater and stone handling, temperature controls, sanitation, barriers, burn response, contraindications, and client communication. General massage training alone does not establish safe thermal-tool use.
