DOPL currently lists a $60 base application fee plus $12 FP-01 and $20 BCI-01 fingerprint-search components; the paper form shows $92. Online payment processing may add up to $3 for a transaction under $100. Confirm the amount shown by DOPL when filing.
Utah licensing
Utah generally requires a Licensed Massage Therapist credential. The standard path uses a 600-hour qualifying education program, the required national and Utah law-and-rule examinations, fingerprint-based criminal-history review, and the DOPL application. A 1,000-hour approved apprenticeship is an alternate education path.
A state license is generally required to practice as a massage therapist, subject to statutory exemptions and Utah's allocation of individual licensure to DOPL.

The current DOPL renewal guide lists no CE requirement for the Massage Therapist credential; the licensing record owns that bounded legal finding.
